Public Health and Freedom of Choice: The Complexities of THR in Pakistan

Pakistan’s efforts to battle smoking have faced significant obstacles. While public health campaigns and awareness programs have been central to addressing smoking, they often clash with the principle of personal choice. These efforts have not achieved the desired impact due to systemic issues like the proliferation of illicit trade and an ineffective track and trace system.

Tobacco Harm Reduction (THR) alternatives – such as vapes, oral nicotine pouches, and heated tobacco products have become crucial. These alternatives offer a significantly less harmful option compared to smoking while delivering nicotine, potentially addressing some of the limitations of current national strategies. They provide a viable path for adult smokers who want to continue smoking. However, THR alternatives are often excluded from the scope of public health policies.

Anti-tobacco groups in Pakistan with their own hidden agendas push for stringent regulations on all nicotine products. They need to acknowledge the increasing scientific evidence that distinguishes between smoking and THR alternatives, treating all nicotine products as equally harmful weakens the potential impact of THR strategies.

While awareness campaigns are important, empowering individuals with accurate and comprehensive information about the risks of smoking and the availability and effectiveness of less harmful alternatives can play a pivotal role. Cultural and social factors such as peer pressure and the desire to fit in, individuals to be accepted by social circles often pick up smoking.

Family dynamics such as when family members, including parents and siblings, smoke, it increases the likelihood of others adopting the habit as well. For many, smoking becomes a coping mechanism to deal with everyday pressures while illiteracy further exacerbates the problem, limiting access to information about the harms of smoking.

Ultimately, a combination of informed choice, accurate information dissemination, and culturally sensitive policies will be key to curbing smoking rates and enhancing the well-being of Pakistan’s population. The journey is challenging, but with a balanced and evidence-based approach, progress is within reach.
